New Patients

All new patients are asked to complete the NEW PATIENT PACKET prior to their visit. Please print, fill out, and bring it to your first appointment along with your insurance card, driver’s license, and any co-pay, co-insurance, or payment that will be due.

  • Check-in time with a completed packet is 15 minutes before your appointment time. This will give us time to input the data prior to your appointment.
  • Check-in time without a completed packet is 20-30 minutes before your appointment time. This will allow enough time to complete the packet and input the data. Please give yourself enough time to complete the paperwork*. Not allowing enough time may, unfortunately, result in rescheduling your appointment, as both parts are very important.

Please bring your current medications and any past medical records related to the visit. Recent labs or x-rays can be requested if we have the release of records form in advance.

If you cannot make your appointment, a 24-hour notice would be appreciated. Thank you.
